[Ssrformat] v1.1 spec

In preparation for the 7th June telecon, here's a list of updates to v1.1 of the spec.

1) Addition of a writable byte to the read header - approved at last telecon
2) Addition of the size of the index block to the index block header - minor change
3) Addition of a read relationship block - to be discussed
4) Changing the index block to support Patricia tries - to be discussed
